  • Advance Care Planning
    Advance care planning is a process that supports adults at any age or stage of health in understanding and sharing their personal values, life goals, and preferences regarding future medical care. The goal of advance care planning is to help ensure that people receive medical care that is consistent with their values, goals and preferences during serious and chronic illness. I am a co lead on "Advance Care Planning and Goals of Care Alberta: a population based Knowledge Translation (KT) intervention study", known as "ACP CRIO". Our funding as an AIHS Collaborative Research and Innovation Opportunities (CRIO) program is for a five year term (April 2013 to March 2018). We are studying the Alberta Health Services-led implementation of a provincial policy on Advance Care Planning (ACP) and Goals of Care Designations (GCD) in Alberta. Partnering with Alberta Health Services and Covenant Health, the KT program employs a knowledge-to-action cycle in order to support a transformational change in practice and healthcare culture. Our objectives are to identify the most effective methods to catalyse use of ACP, and to determine the impact on health resource utilization and clinical outcomes.
